First Visit Client Forms
Please download, print, read, and sign all forms for your first appointment. If you do not have a printer, please notify Dr. Ryan and he will provide you a copy at his office to fill out before the first appointment. Please allow 30 minutes to fill out the forms at his office. Note: the Credit Card Authorization Form is only needed if you want Dr. Ryan to have a credit card on file to bill you for his services. However, all other forms are required. Thank you!
